Medication Management Follow-Up

Focused follow-up visit to monitor progress, adjust medications, and support your treatment goals.

Brief Follow-Up

15 minutes-Established & Clinically Stable Patients only

Reserved for patients who are clinically stable at the provider’s discretion.

Extended Follow-Up

45-60 minutes

Designed for complex cases, therapy integration, medication transitions, or extended care needs.

Payment Policy

• Payment is due at the time of service.

• HSA/FSA cards accepted

• A superbill can be provided upon request for potential out-of-network reimbursement.

Cancellation Policy

Appointments canceled with less than 24 hours’ notice, or missed appointments, will incur a fee equal to 50% of the scheduled visit cost.

Repeated missed appointments may result in reassessment of continuation of care within the practice.

This policy protects appointment availability for all patients.
